Immunization Requirements

Please provide a copy of your student's immunization record to the school registrar/ school nurse for review.

Your child must be current in their vaccinations to attend school.

Please complete the health information form below and return it with a copy of your immunization record to the school registrar or school health office.

If your student has a medical condition that will require adult support at school or medication to be administered, please go to the Health Forms tab to download the appropriate forms for your medical provider to complete.
